He had killed the Old Man and hid safely under the planks. As policemen came for inquiry everything was perfect. They were there just for the sake of inquiry. They were sitting on chairs and just chit chatting. But the narrator thought that they knew everything and laughing at him just to make him confess his guilt himself.

When the narrator heard the heart beat going louder and louder he attacked the old man. He pulled the heavy bead over him and the old man died of suffocation.

I don’t think that the narrator was mad. Because he repeatedly calls himself that he was not a mad man but a man with acute sense of hearing. He might have been suffering from some dangerous disease or similar illness. He clearly mentions that he is not after the old man or his wealth and gold but what irritates him is the Evil Eye of the old man. Whenever he sees them his blood boils. Because of this reason the narrator develops intense hatred and plans to get rid of the eye as soon as possible. He tries till the eighth day and gets success.

If he had been mad, he would not kill him and hide the body systematically under the planks. He wouldn’t reveal the truth easily. So, the person seems idiosyncratic but cannot be called mad.
